Back pain and footwear: what shoes can and cannot do

Back pain is common and may involve muscles, joints, nerves, injury or another health condition. Footwear type alone rarely explains every symptom. Barefoot shoes are not a treatment for back pain and cannot guarantee improved posture.

Back pain has many causes

The overall clinical picture matters when pain is new or persistent. Most ordinary back pain improves, but some symptoms need urgent assessment.

When to seek urgent help

  • numbness around the genitals or anus
  • difficulty controlling the bladder or bowel
  • weakness or numbness in both legs
  • severe pain after major injury or with fever

What footwear may actually affect

Shoes may affect walking comfort, pressure under the foot and confidence with each step. A wide toe box and flexibility suit some people but not everyone. Stop a footwear change if it worsens pain and seek professional advice.

How to try shoes when symptoms are present

  • choose the correct length and width
  • keep the heel secure without rubbing
  • introduce a new footwear type gradually
  • monitor pain during and after wear

What guidance says

NICE does not recommend foot orthotics or rocker sole shoes as routine management for low back pain. A prevention review also found low-quality evidence of no protective effect from insoles. Activity and treatment should match the diagnosis and advice from a health professional.
